A multi-media toolkit for life story work
'Portrait of a Life' includes two DVD's and a CD ROM, a written guide and photocopiable resources.
DVD (1) is a training DVD that includes an expert evidence base, covering the risks, benefits, consent, ethics and more in addition to real practice examples.
DVD (2) is a 14 minute film which is a moving portrayal of the perspective of a person living with dementia who is a resident in a care home.
The CD ROM enables the user to access printable information and resources as Paper Document Files (PDF) including CARER model documentation for Life story work and partnership working with family, carers, an example consent form, example evaluation form, other resource material and an e-learning guide to developing life story journals.
The detailed written guide reflects the same information contained within the training DVD. The practical approaches within the toolkit support the production of life story boards, electronic journals, memory boxes and more for people living with dementia and other mental health needs in a variety of care settings.
The toolkit enables those using it to understand and deliver true person-centred care through transforming care planning and seeing the person first.
